This Credit Card Authorization Form permits Land and Sea Travel, LLC to debit/credit the listed account for travel services to Travel Suppliers on or after the date of this agreement. Please read the Cancellation/change penalties that may apply to changes or cancellations of your travel.

 

Please verify all legal names (as appears on passport or government ID), birth dates and ages of guests are correct. 

Families or individuals traveling together will only need to fill out one authorization form.   If individuals would like to make payments on separate credit cards then separate authorization forms must be completed for each individual card holder.

Please make sure it is the age of the guest at time of travel. If names are not correct, you may incur change or service fees.
